Terms Of Service
Eligibility for benefits begins when membership fees are paid. This fee is assessed yearly, and membership is valid one year from payment. Family membership is defined as your spouse and children living in your home or any other legally defined family member living in the same household.
Your membership in our discount medical plan is an agreement that provides you access to participating Providers to offer you discounted services.
This is not an indemnity insurance plan. Patient Care Solutions Inc. does not reimburse your provider for service and fees paid.
In addition it cannot be used as payment for deductibles or insurance plan co-pays. Members are responsible for full payment of all services rendered from any contracted provider.
A yearly fee is required to hold an active membership and will renew yearly until canceled. All members are free to terminate their membership with Patient Care Solutions Inc. at any time with 30 days written notice. No cancellation fee required.
I declare that I would like to enroll in Patient Care Solutions Discount Medical Plan. I have read and understand the terms of this membership agreement as listed on this form. I understand this membership is not medical insurance, but contractually allows me to access a discounted fee schedule for the services I receive at this office and any office that is a member of Patient Care Solution. Failure to abide by the terms of this agreement may result in termination of my membership.
